Vocational/Avocation Directory 2008 Information Submission for D6630 Members

The Vocational/Avocation Directory is for the exclusive use of District 6630 Rotarians and may not be used for any other purposes. The directory will be listed in a password-protected section of the District web site. It will be both searchable, and include a PDF version that may be downloaded and printed.

Use of this Directory as a commercial mailing list or for other commercial purposes is strictly prohibited. Participants will not disclose any of the information contained in the directory to third parties.

As a participant, I agree to hold Rotary Interational and District 6630 harmless from any claims whatsoever.

The directory will be updated quarterly (we will try to do it more often in the beginning, but your entry will not appear immediately).

If you make a mistake, or want a change, please fill out the form again and resubmit.

Please submit information for the 2008-2009 Rotary year. Before submitting the form below:

Please make sure you have all information ready to enter in a single submission. Partial submissions should NOT be made.
Please do not use ALL CAPS (except for entering the state as OH).
Please use hyphens for separating phone numbers, as in 800-555-1212.
